    Responsibilities
    - Deliver coordinated nursing care for an individual patient or assigned group according to the nursing process and established standards
    - Collect relevant data, analyze for diagnoses and care needs, and develop a plan of care
    - Implement the plan, coordinate care delivery with the interdisciplinary team, and promote a safe, healing environment
    - Monitor patient progress, adjust interventions as needed, and evaluate outcomes
    - Supervise and direct activities of assigned nursing staff, delegating tasks appropriately
    - Participate in discharge planning and provide education to patients and families

    Requirements
    - Active Registered Nurse license in California
    - Minimum two years of recent nursing experience in a relevant clinical setting
    - Basic Life Support (BLS) certification approved by the employer (CPR acceptable per policy)
    - EPIC EMR experience within the last 12 months
    - Bachelor’s Degree in Nursing (BSN) preferred
    - Ability to work in a home healthcare - hospice environment
