Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Collect data on physical, psychological, social, spiritual, and cultural factors and incorporate that data into the Plan of Care.
- Maintain up-to-date patient records with visits, contacts, problems, plans, actions and goals; reflect changes promptly.
- Educate patients and families regarding care, medications, home safety, and self-care; adapt teaching to age, condition, culture, and readiness.
- Coordinate care with the interdisciplinary team and supervise/provide clinical directions to HHA/STNA/LVN as appropriate; arrange referrals when needed.
Requirements
- Current, active Registered Nurse license in the attending state (NC) - Required
- CHPN certification - Preferred
- CPR certification - Required
- Current driver’s license and reliable transportation - Required
- 1–2 years RN experience in a clinical care setting - Required
- 2+ years Home Health experience - Required
- Education: Associate Degree in Nursing or Nursing Diploma program; BSN preferred
