Responsibilities
- Provide direct nursing care for assigned patients according to the nursing process and established standards.
- Collect relevant data, assess health status, develop and implement an individualized care plan, and evaluate outcomes.
- Coordinate care with interdisciplinary team members and ensure safe, effective delivery of services.
- Supervise and direct the activities of various levels of nursing staff; delegate duties appropriately.
- Educate patients and family members; participate in discharge planning and transition to home care.
- Document care in the designated electronic health record and monitor for quality and safety throughout the shift.
Requirements
- Active Registered Nurse license in California (state of assignment)
- Minimum 2 years of recent nursing experience in the field
- Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification; Hospital Fire and Life Safety (HLFS) certification
- EPIC electronic health record experience
- Bachelor of Science in Nursing (BSN) preferred
- Ability to work in a home healthcare – hospice setting
