Responsibilities
- Provide direct, compassionate nursing care to residents in a memory care setting within a long-term care framework
- Assess, plan, implement, and evaluate individual care plans; monitor vital signs and clinical status
- Administer medications and treatments as prescribed; document accurately in the electronic MAR
- Respond promptly to changes in resident condition; assist with emergency interventions as needed
- Collaborate with an interdisciplinary team and communicate effectively with families and care partners
- Maintain safety, infection control, and high-quality care standards; document observations and ensure continuity of care
Requirements
- Active Registered Nurse license (state of Vermont or eligible to practice in VT)
- Ability to work night shift: 11:00 p.m. – 7:00 a.m.
- Minimum 40 hours per week guaranteed
- Willingness to work in a long-term care/memory care setting
- Experience in LTC/memory care preferred but not required for eligibility
