    Territory Includes:

    Altoona, Irwin, Meadville, New Castle, Pittsburgh, Somerset, and Wexford

    Eligible for a Company Fleet Vehicle

    Schedule: Monday-Friday

    What You'll Do

    As a Hospice RN - Regional Support, you'll provide direct patient care while serving as a floating clinician assigned to hospice branches based on census needs, growth opportunities, or staffing support.

    Responsibilities include:

    • Managing and coordinating hospice patient care

    • Performing comprehensive patient assessments

    • Developing and implementing individualized care plans

    • Educating and supporting patients, families, and caregivers

    • Collaborating with interdisciplinary hospice teams

    • Providing compassionate care that promotes comfort, dignity, and quality of life

    As a Regional Support Hospice RN, You Will:

    • Deliver high-quality, compassionate nursing care for hospice patients
    • Manage and implement individualized plans of care
    • Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
    • Assess and address patients' physical, emotional, spiritual, and environmental needs
    • Accurately document all care provided and ensure compliance with hospice protocols
    • Educate families and caregivers on care instructions, medications, and safety
    • Participate in the on-call rotation and provide after-hours care as needed
    • Collaborate with physicians, referral sources, and the care team
    • Assist with referrals, regulatory documentation, and payer compliance
    • Travel to various Gentiva hospice locations as assigned to fill urgent needs

    Schedule & Travel:

    This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities. Travel is required and may include:

    • Regional travel with overnight stays based on assignment needs.

    A company fleet car is provided for eligible employees.

    About You

    Qualifications - What You'll Bring:

    • Current, unencumbered RN license in the state of employment
    • Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
    • Current CPR certification
    • Valid driver's license, auto insurance, and reliable transportation

    Preferred Experience (Not required):

    • Hospice and end-of-life nursing experience
    • Skilled in clinical assessments and compassionate communication
    • Strong critical thinking, flexibility, and problem-solving skills
    • Comfortable working in varied environments with shifting needs
    • Proficient in documentation and electronic health records
    • Calm and supportive in crisis situations; skilled at family engagement
