Are you a compassionate, highly skilled Travel RN looking for your next assignment in Vermont? We are seeking an experienced Travel Registered Nurse (RN) – Long-Term Care (LTC) to join our dedicated interdisciplinary team in Glover, VT.
In this role, you will deliver high-quality, resident-centered nursing care in a skilled nursing and long-term care environment. You will focus on managing chronic conditions, conducting comprehensive health assessments, administering treatments, and collaborating closely with physicians, therapists, and families to maintain the comfort, safety, and dignity of our residents.
Weekly Schedule: 40 hours per week (Combination of 12-hour and 8-hour shifts)
Shifts:
Two (2) 12-hour shifts: 6:45 AM – 7:15 PM
Two (2) 8-hour day shifts
Weekend Requirement: Every other weekend
Perform comprehensive physical and health assessments, closely monitoring residents for changes in condition.
Administer medications and manage IV therapy according to prescribed orders and facility protocols.
Provide skilled wound care, skin integrity management, and pressure ulcer prevention strategies.
Identify and escalate acute clinical changes immediately, updating care plans in collaboration with the interdisciplinary team.
Coordinate care plans alongside physicians, physical/occupational therapists, and social workers.
Maintain timely, accurate, and thorough documentation of all care, treatments, and resident responses in the Electronic Health Record (EHR) system.
Provide compassionate disease management education and safety guidance to residents and their families.
Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
Licensure: Active and unencumbered Registered Nurse (RN) license valid in the State of Vermont (or active Compact/NLC RN license).
Basic Life Support (BLS) certification required.
Certified Geriatric Nurse (CGN) or similar gerontology credential preferred.
Proven clinical experience in a long-term care (LTC), skilled nursing facility (SNF), or geriatric care setting.
Demonstrated proficiency with electronic medical documentation systems.
Strong clinical assessment, communication, and care coordination skills.
Flexibility and adaptability to seamlessly step into a travel nursing assignment.
