Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients using established standards and the nursing process
- Supervise and direct activities of varying levels of nursing staff; coordinate care with other disciplines
- Collect relevant data, perform assessment, formulate a care plan, implement interventions, and evaluate progress
- Participate in discharge planning to support continuity of care and safe transitions
- Delegate appropriately, coordinate duties of the healthcare team, and provide education to patients, families, and team members
- Maintain a safe, healing, and respectful environment; communicate clearly with the patient, family, and treatment team

Requirements
- Licensure: Registered Nurse (RN) license in the state of practice (California) required
- Certifications: CPR or BLS certification required; Basic Life Support (BLS) certification per policy required; Hospital Fire and Life Safety (HLFS) required; ACLS and PALS certifications required per policy
- Experience: At least two years of recent nursing experience in the field
- Education: BSN preferred
- Ability to work in a home healthcare - hospice setting
- Spanish speaking is a plus
