Join a leading healthcare provider as a Care Management RN in Boston, MA. This contract position offers an opportunity to work in a dynamic environment where you will utilize your expertise in long-term care (LTC) and skilled nursing facility (SNF) settings. As a Care Management RN, you will be responsible for assessing, planning, implementing, monitoring, and evaluating the options and services required to meet patients' health needs. You will work collaboratively with a team to ensure quality, cost-effective outcomes for 18-22 patients on the Menino Campus. Your role will involve managing length of stay (LOS) and promoting efficient utilization of resources.

Key Responsibilities:

• Conduct assessments of physical and mental impairments.
• Utilize methods for understanding the characteristics of illness and expected treatment.
• Determine medical necessity and appropriateness of admission using InterQual screening.
• Manage patient needs and coordinate care effectively.

Requirements:

• Required Experience: 3+ years in care management with a strong background in current medical/surgical patterns of practice and acute inpatient care.
• Preferred Experience: 5+ years with Utilization Management (UM) and Care Management (CM), EPIC experience.
• Required Certifications: BSN.
• Preferred Certifications: CCM or related certification.

Shift details include 10-hour shifts, working 4x10s with no set day off, and every 6th weekend as needed. Please note that floating may be required based on unit needs. Local candidates residing less than 50 miles from the facility will be compensated at a local rate, which is $10 less than the posted bill rate.
