Shift: Days, 8:00 AM–4:30 PM
Schedule: Monday–Friday
Weekend: Rotating weekends required
Experience:Recent Acute Care Discharge Planning experience required
EMR/Systems: Meditech, Midas, WellSky
Patient Ratio: 1:25, varies
Floating: May float to any unit
Team: RN, Social Worker, CMA
On Call: None
Dress Code: Business casual
Contract: 12 weeks
Key requirement for candidates: Recent acute-care discharge planning/case management experience is required.
