**Exciting Opportunity for RN Case Manager - Travel Position** **Job Description:** - Join our dynamic team as an RN Case Manager! - Coordinate the management of patient care to ensure the best outcomes. - Optimize resource utilization and enhance service delivery. - Comply with external review agencies to maintain high standards. - Provide ongoing support through comprehensive assessments and care planning. - Implement and evaluate care plans tailored to individual patient needs. - Advocate for patients through effective resource utilization and discharge planning. - Address the holistic needs of patients to promote satisfaction and continuity of care. **Requirements:** - Must possess BLS certification from the American Heart Association. - Minimum 2 years of recent RN Case Management experience in LTAC or STACH settings. - Experience with MCG and discharge planning across acute care settings. - Strong collaboration skills within an interdisciplinary team. - Valid Texas RN or multi-state RN license. **Position Details:** - Shift details: Monday - Friday, 8am-5pm. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
