We are seeking a dedicated and experienced Registered Nurse (RN) for a Case Manager position at a leading healthcare facility in Randallstown, MD. This contract role focuses on providing exceptional case management services, particularly in the Emergency Department and medical inpatient units.

Key Responsibilities:

• Manage case loads for adult and geriatric patients, coordinating discharge planning and transitions of care.
• Work collaboratively with Social Work to ensure effective patient management.
• Support approximately 1.5-2 units during crisis staffing, primarily in a 20-bed unit setting.
• Conduct assessments and develop care plans for medically complex patients in Med-Surg and Critical Care.

Requirements:

• Active Maryland RN license is required upon submission.
• A minimum of 2 years of hospital inpatient RN Case Management or discharge planning experience is essential.
• Experience in Med-Surg and/or Critical Care discharge planning is mandatory; therapy-only backgrounds will not be considered.
• BLS certification is required.
• Ability to work independently after approximately 3 days of orientation.
• First-time travelers are welcome, provided they meet the required experience.

Shift Information:

• Day shift, 5x8 hours from 08:00 to 17:30, Monday through Friday.
• Agency staff may be scheduled up to every other weekend based on staffing needs.
• One holiday per year is required, consistent with staff expectations.

Additional Information:

• One pre-approved time-off request is permitted per contract; the system will auto-approve 14 days or less of RTO.
• No RTO on hard-to-fill days such as Easter, Mother's Day, and Father's Day.
• Local candidates are accepted.
• Charting system used is Cerner.
• Business casual scrub color is expected.
