Job Responsibilities
- Coordinate and manage patient care throughout hospitalization.
- Perform comprehensive patient assessments and discharge planning.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
- Review medical necessity using InterQual criteria.
- Facilitate utilization review and resource management.
- Coordinate post-acute care services and community resources.
- Document patient care activities accurately in Meditech and Midas.
- Ensure compliance with hospital policies, CMS guidelines, and regulatory standards.
- Advocate for patients to ensure appropriate, timely, and cost-effective care.
Minimum Requirements Active Registered Nurse (RN) license. Recent acute care (hospital) Case Management experience required. Proficiency with: Meditech Midas InterQual Strong knowledge of utilization review, discharge planning, and care coordination. Excellent communication and interdisciplinary collaboration skills. Preferred Qualifications Previous travel nursing experience. Experience in a high-acuity acute care hospital. Familiarity with HCA workflows and documentation standards. Required Certifications Active RN License BLS (AHA) (if required by facility) Additional certifications as required by HealthTrust or the facility.
