POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care.
MINIMUM REQUIRED QUALIFICATIONS –
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00am-4:30pm with rotating weekends
SYSTEMS – Meditech
START DATE – 9/14/2026
