Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. MINIMUM REQUIRED QUALIFICATIONS – MA RN license. MA conditional license will be considered; must be in the process of obtaining a perm license OR apply for a perm license upon offer. AHA BLS 2 years of case management experience 6-months discharge planning Case Management experience in an Acute Care hospital (ideally be a large organization/teaching hospital with 500 beds or more) LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – M-F, Days. Dependent on operational need, may be 4x10's or 5x8's.
