POSITION SUMMARY 
RN Case Manager or Social Worker

POSITION DUTIES 
Discharge planning coverage for ER/ICU/Med Surg units. This does not include Utilization Management

MINIMUM REQUIRED QUALIFICATIONS 
3 or more years of Hospital Case Manager experience
Case Management Certification preferred but not required
LENGTH OF ASSIGNMENT 13 Weeks

SHIFT / HOURS PER WEEK 
Days, M-F 8am-4:30pm (every fourth weekend)
