Description

American Traveler is hiring an experienced RN Case Manager for an acute hospital Case Management position requiring a minimum of 4 years of experience and Epic proficiency, working Monday through Friday day shifts.

Details

  • Acute hospital Case Management/Utilization Review unit
  • Patient population requires comprehensive case management, utilization review, and discharge planning
  • Average daily census of 20 patients on weekdays and 70 on weekends
  • Schedule is 5x8-hour day shifts, Monday through Friday, 8:00 AM to 2:30 PM
  • No on-call responsibilities
  • Epic EMR is used and proficiency is required

Requirements

  • Active RN license in North Carolina (or compact license with NC as primary state) currently in hand
  • Current BLS certification required
  • ACM or CCM certification preferred
  • Minimum of 4 years of RN experience required
  • Acute hospital experience required
  • Epic EMR experience required
  • BSN preferred; graduation from an accredited school of nursing required
  • Two professional references required for consideration
  • PDF copies of Driver's License and Social Security Card required for consideration

Additional Information

  • Conduct comprehensive patient assessments and develop individualized care plans to support continuity of care and cost-effectiveness
  • Coordinate utilization review and discharge planning to promote quality patient management and satisfaction
  • Must be highly organized with strong attention to detail and the ability to adapt to frequent change while maintaining compliance with regulatory and departmental guidelines
  • Orientation is 24 hours
  • First-time travelers will not be considered for this position
  • Candidates must reside more than 50 miles from the facility to be eligible
  • Any requested time off must be submitted prior to acceptance of an offer to be considered
