Duties:
Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required.

Skills:

• 2 years of acute care experience strongly preferred
• Case Management, Managed Care.
• In Patient Case Management is required
• Must have exp. with Sub- acute pts.

• BSN required
• RN license required
• CCMC preferred
