Description

American Traveler is hiring an experienced RN Case Manager for a teaching hospital, requiring an active MA RN license and 3+ years of acute care case management experience.

Details

• Case Management department within a 298-bed teaching hospital
• Manages healthcare needs and coordinates care across the continuum for all patient populations
• Performs admission and concurrent utilization review, discharge planning, and transition care coordination
• Day shift schedule, 8-hour shifts, 7:00 AM-3:30 PM, five days per week
• 10 and 12-hour shift schedules are not available for this position
• No weekend requirements outside of potential holiday coverage
• No on-call requirements

Requirements

• Active MA RN license required in hand at time of profile submission (compact license not accepted)
• BLS certification required (AHA or ARC)
• Minimum 3 years of acute care clinical experience in areas such as internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology
• Clinical experience in rehab, home health, or long-term care settings will also be considered
• Ability to perform comprehensive admission and discharge risk assessments related to quality, financial, payer benefits, and risk management
• Knowledge of regulatory and accreditation standards (Joint Commission, DPH, HealthGrades) preferred
• CCM or ACM certification preferred
• RN-BC certification in Case Management preferred
• Meditech Expanse EMR experience highly preferred
• Knowledge of Medicare, Medicaid, and third-party payer reimbursement required
• Strong computer literacy with ability to manage multiple applications simultaneously

Additional Information

• Utilizes a multidisciplinary team approach to negotiate, procure, coordinate, and monitor patient services and resources
• Participates in facility performance improvement initiatives
• Must have own vehicle for this position
• Candidates must reside more than 50 miles from the facility to be considered
• Any requested time off must be submitted and verified at the time of profile upload; no more than 5 days of RTO will be accepted
• Scheduling requests cannot be made or approved after offer
• Must be available to work two designated holiday blocks if scheduled, including Thanksgiving weekend, Christmas Eve through New Year weekend
• Strong interpersonal, negotiation, and collaboration skills required for working with providers, patients, families, and insurance organizations
• Ability to work independently with strong organizational and analytical skills
• Knowledge of local community resources required
