Description

American Traveler is hiring an experienced RN Case Manager for a hospital serving a culturally diverse patient population, requiring 3 years of clinical nursing experience and an active RN license.

Details

  • Work as an Outpatient/Inpatient Nurse Case Manager within a hospital setting serving a large referral area
  • Focus on specialty clinics requiring dedicated Case Management attention for high-risk patient populations
  • Interact with patients of all ages, from newborn to geriatric
  • Coordinate care for patients dealing with abuse, chronic pain, and frequent utilization issues
  • Evaluate ED patients for medical necessity and care coordination needs
  • Collaborate with an interdisciplinary team including advanced practice clinicians and physicians
  • Schedule is Monday to Friday, 8am-5pm with callback required on weekends and holidays
  • 13-week assignment length

Requirements

  • Active, unrestricted RN license (AZ or Compact accepted)
  • Current BLS certification required
  • Additional certifications may include ACLS, PALS, NRP, or ATT
  • Associate's or Bachelor's Degree in Nursing required
  • Minimum 3 years of clinical nursing experience required
  • At least 1 year of Outpatient/Inpatient setting experience required
  • Demonstrated knowledge of case management, contract health, and utilization review processes required
  • Fingerprinting must be completed and cleared prior to start

Additional Information

  • Facilitate care transitions from admission through discharge, ensuring continuity of care and coordination of post-hospital referrals
  • Serve as a resource for nursing, physicians, and ancillary departments on transitional and regulatory issues
  • Work with culturally diverse patient populations facing barriers such as illiteracy, substance abuse, domestic violence, and mental health challenges
  • Assist patients with mental health issues in accessing and navigating available resources
  • Assess potential ED admissions and propose safe discharge plans in collaboration with physicians
  • Facility offers shared contractor housing only, which is very limited; no pets or family members allowed per hospital policy
  • Housing application must be submitted at least 2 weeks prior to start
  • Orientation occurs every 2 weeks
  • COVID and flu vaccine exemptions may be accepted with a completed declination form
  • Located in a rural area on the Navajo and Hopi Reservations, offering a unique and culturally rich community experience
