Description

American Traveler is hiring an RN Case Manager for a Discharge Planning role in Acute Care requiring active MA RN license and 2+ years of case management experience.

Details

• 13-week assignment in the Case Management department of a large academic acute care hospital
• 40 hours per week on day shift, Monday through Friday, scheduled as either 4x10s or 5x8s depending on operational need
• Shift hours are 07:30-18:00
• Uses Epic for charting

Requirements

• Active MA RN license required; conditional license considered if in process of obtaining permanent licensure
• AHA-issued certifications required
• Minimum 2 years of case management experience required
• At least 6 months of discharge planning case management experience in an Acute Care hospital required
• Experience in a large teaching hospital with 500+ beds strongly preferred
• Flu vaccination required with no religious or medical exemptions allowed

• Responsible for discharge planning and case management duties within an acute care hospital setting
• Local candidates accepted with reduced rate if residing within 60 miles of Boston, MA
