**Join Our Team as an RN Case Manager with Utilization Review Experience!** **Job Description:** - **Position Summary:** - As a Registered Nurse Non-Certified Case Manager, you'll be a vital part of our multidisciplinary team. - Collaborate with physicians and healthcare professionals to create and implement personalized care plans for patients. - Monitor patient progress, resolve barriers to care, and actively participate in discharge planning. - **Minimum Qualifications:** - Montana Licensure (Registered Nurse) - American Heart Association BLS certification - Minimum of 2 years of nursing experience - Must have Utilization Review experience - **Preferred Qualifications:** - Bachelor’s Degree in Nursing - 2 years of acute care hospital experience as a case manager - **Essential Job Functions:** - Conduct comprehensive assessments of members and determine eligibility for case management. - Develop and implement tailored case management plans in collaboration with patients and healthcare providers. - Perform ongoing monitoring of care plans and suggest modifications as needed. - Manage a consistent case load with regular outreach to members. - Promote integration of services, including behavioral health and long-term care. - **Knowledge, Skills, and Abilities:** - Demonstrate sound judgment and maintain a professional demeanor at all times. - Thrive in a busy and fast-paced environment. - Exhibit strong interpersonal, verbal, and written communication skills. - Maintain confidentiality while exercising discretion and sensitivity. - Proficient in computer applications, including MS Office, EMR, and standard office equipment. - Possess strong organizational skills and the ability to prioritize effectively. - Exhibit strong emotional intelligence and teamwork skills. - **Schedule Requirements:** - Regular and sustained attendance is essential. - Flexibility to work beyond a standard 40-hour workweek, including weekends and after-hours shifts, may be required. - On-call work may be necessary to address immediate organizational, patient, or employee needs. - **Physical Requirements:** - Repeatedly lift up to 50 pounds. - Ability to maintain various postures for extended periods. - Frequent walking and coordination required within the work area. - Flexibility in bending, twisting, and reaching as necessary. - Engage in fine finger movements and maintain close visual acuity for diverse tasks. - **Exposures:** - Regular contact with bloodborne pathogens and other healthcare-related risks. - Exposure to various chemicals and medications. - Manage emotionally challenging situations, including distressed patients and complex family interactions. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
