Registered Nurse (RN) Case Manager – Home Health

Locations: Bradford, VT | Randolph, VT | Springfield, VT | White River Junction, VT | Lebanon, NH
Shift: Full-Time | Days
Contract: 16 Weeks
Weekly Gross: $2825

 Mandatory Qualifications for this Job

  • Active Vermont RN, New Hampshire RN, or Compact RN License
  • Graduate of an Accredited Nursing Program (ADN or BSN)
  • 2+ years of Home Health RN experience
  • Current BLS Certification
  • Home Health Case Management experience
  • Proficiency with EPIC EMR
  • Strong patient assessment, care planning, and documentation skills
  • Excellent communication and teamwork skills
  • Valid driver's license and reliable transportation (preferred)
  • Ability to work independently and manage a home health caseload

 Roles and Responsibilities

  • Conduct comprehensive patient assessments in the home setting
  • Develop and update individualized care plans
  • Coordinate care with physicians and the interdisciplinary team
  • Monitor patient progress and report changes in condition
  • Provide skilled nursing care, medication management, and patient education
  • Educate patients and caregivers on disease management and self-care
  • Maintain accurate documentation in EPIC EMR
  • Participate in care conferences and discharge planning
  • Promote patient safety, independence, and quality care
  • Ensure compliance with home health policies and regulatory standards

This is a 16-week, full-time Day Shift Home Health RN Case Manager opportunity with a weekly gross pay of $2,825 and an ASAP start. Candidates must have an active Vermont RN License, New Hampshire RN License, or Compact RN License, a minimum of 2 years of Home Health RN experience, current BLS certification, and experience in Home Health Case Management. Proficiency with EPIC EMR, strong assessment and care coordination skills, and the ability to work independently are required. A valid driver's license and reliable transportation for home visits are preferred. Only candidates meeting all mandatory qualifications will be considered for submission.
