Description

American Traveler is hiring a Home Health RN Float requiring an active MA RN license and at least 1 year of certified home health and case management experience.

Details

• Home health setting covering the Lynn, Nahant, and Saugus areas within the North Region
• 13-week assignment with an 08:00-16:30 shift schedule
• Weekend rotation required once every 6 weeks
• Ability to travel to patient homes as assigned
• Uses Epic for electronic charting and documentation

Requirements

• Active MA RN license required, no pending or conditional licenses accepted
• Minimum 1 year of experience specifically within a certified home health agency
• Minimum 1 year of case management experience within a certified home health agency setting
• AHA-issued certifications only
• Flu vaccine required with no religious or medical exemptions allowed
• Primary source verification of license required through both NURSYS and BON

Additional Information

• Responsible for assessing patient conditions, implementing care plans, and educating patients and families in the home setting
• Performs admission processes and case management duties per organizational policies and standards
• Requires thorough and timely electronic documentation and interdisciplinary communication
• Care delivered in accordance with community health practice standards and organizational philosophy
• Local candidates residing within 60 miles of Massachusetts General Hospital in Boston will be considered at a reduced rate
• Uses UKG/Kronos for timekeeping
