Home Health Registered Nurse (RN) – 13-Week Contract in Newton, Massachusetts

In this role, you will provide skilled nursing services in patients’ homes, coordinate individualized plans of care, and support patients and families across the North Region, including Lynn, Nahant, and Saugus. This travel nursing opportunity is based in Newton, Massachusetts, a welcoming Boston-area community known for its vibrant neighborhoods, green spaces, and convenient access to the region.

Home Health RN Job Details

• Weekly Estimated Pay: $2,500–$2,630
• Location: Newton, Massachusetts; community-based home health visits throughout assigned service areas
• Assignment Dates: November 1, 2026–January 30, 2027
• Duration: 13-week contract
• Schedule: Day schedule, 8-hour shifts; scheduled window of 8:00 AM–4:30 PM
• Hours: 40 hours per week
• Weekend Rotation: Approximately once every 6 weeks
• Position Type: Travel nursing assignment with local home health patient visits

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license eligible for practice in Massachusetts
• At least 1 year of experience in certified home health care
• At least 1 year of case management experience
• Experience must be obtained specifically within a certified home health agency; hospice, private-duty care, or standalone case management experience does not meet the home health experience requirement
• Ability to travel to assigned patient cases throughout the service area
• Strong clinical assessment, organization, communication, and care coordination skills
• Comfort with timely electronic documentation and interdisciplinary collaboration

Home Health Nursing Responsibilities

• Assess patients’ health conditions, needs, risks, and available support systems in the home setting
• Develop, implement, and evaluate individualized plans of care
• Complete admission processes and establish appropriate home health services
• Provide skilled nursing care and ongoing monitoring based on patient needs and established standards of practice
• Perform home health case management duties, coordinate services, and communicate changes in patient status
• Educate patients and families regarding disease management, medications, treatment plans, and self-care
• Maintain thorough and timely electronic clinical documentation
• Collaborate with physicians and interdisciplinary care team members to promote safe, effective patient outcomes
• Participate in scheduled weekend rotations and travel between assigned patient cases as needed
