Job Requirements

• Active Registered Nurse (RN) license
• Minimum 1 year of certified home health or home care experience required
• Minimum 1 year of case management experience required
• Experience must be within a certified home health agency
• Ability to travel to assigned patient cases as needed throughout the Newton and greater Boston area
• Strong electronic documentation, clinical assessment, and interdisciplinary communication skills
• Weekend rotation approximately once every 6 weeks

Responsibilities

• Assess patient conditions and develop appropriate nursing interventions in the home setting
• Implement and update individualized care plans to support patient recovery and independence
• Provide patient and family education, support, and ongoing care guidance
• Complete admission processes and case management responsibilities
• Maintain accurate, timely electronic health record documentation
• Communicate effectively with the interdisciplinary care team, including providers and support staff
• Deliver care in accordance with organizational policies, home health standards, and community health best practices
