**Join Our Dynamic Team as a Physical Therapist!** **Job Summary:** We are searching for a passionate and talented Physical Therapist (PT) to enhance our interdisciplinary team in a Skilled Nursing Facility (SNF). In this rewarding role, you will: - Evaluate residents' physical conditions and movement limitations. - Develop and implement personalized treatment plans to meet individual goals. - Deliver evidence-based interventions to improve strength, coordination, balance, and mobility. - Collaborate with a diverse team, including physicians, nursing staff, occupational and speech therapists, and family members. - Monitor patient progress, adjust therapy plans as necessary, and document care in compliance with state and federal regulations. - Educate patients and caregivers on safe mobility techniques and home exercises. - Supervise physical therapist assistants (PTAs) and aides when needed. - Maintain a safe and clean work environment. **Qualifications:** - Graduate of an accredited Physical Therapy program. - Current state license to practice as a Physical Therapist (PT). - CPR certification (preferred or required depending on the facility). - Experience in a Skilled Nursing Facility or geriatric rehab setting is preferred. - Excellent communication, documentation, and interpersonal skills. - Ability to work independently or as part of a multidisciplinary team. **Work Environment:** - Skilled Nursing Facility (SNF) – inpatient, long-term care, and short-term rehabilitation. - Responsibilities may include lifting, standing, walking, and assisting patients with transfers and mobility. **Shift Information:** - 9 AM to 5 PM **Minimum Years of Experience:** - Less than 1 year **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
