Key Responsibilities
Complete functional assessments and develop individualized treatment plans for residents
Provide therapeutic interventions focused on activities of daily living (ADLs), mobility, and safety
Communicate resident progress and recommendations with the interdisciplinary team and families
Collaborate with COTAs and Rehab Aides to ensure consistent, high-quality care
Foster a positive, resident-centered rehabilitation environment
Required Qualifications
Degree from an accredited Occupational Therapy program
Active Oregon Occupational Therapy license or eligibility to obtain
CPR certification or ability to obtain prior to start
Strong communication, clinical reasoning, and teamwork skills
Preferred Experience / Skills
Experience in a skilled nursing facility (SNF), post-acute, or long-term care setting
Familiarity with Nethealth documentation software
