Job Description

Ardor Health Solutions is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Jamestown, Virginia.

Position Details

  • Setting: Skilled Nursing Facility (SNF)
  • Location: Jamestown, Virginia
  • Schedule: Full-Time | Monday–Friday
  • Patient Population: Primarily geriatric and post-acute rehab patients
  • Start Date: ASAP

Responsibilities

  • Evaluate and treat patients with orthopedic, neurological, and medically complex conditions
  • Create individualized treatment plans focused on mobility, strength, balance, and functional independence
  • Collaborate with interdisciplinary rehab and nursing teams
  • Complete daily EMR documentation and progress notes
  • Educate patients and families on treatment plans and discharge recommendations
  • Maintain productivity standards while delivering quality patient care

Qualifications

  • Graduate of an accredited Physical Therapy program
  • Active Virginia PT license or ability to obtain one
  • SNF or geriatric experience preferred
  • Strong communication and teamwork skills
  • New grads welcome depending on facility support and mentorship availability
