Job Summary:
Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) is needed for a day shift role beginning October 12, 2026, through January 9, 2027. This position involves documenting patient care in the designated electronic health record system and providing therapy services in a healthcare facility setting. The ideal candidate will be familiar with documentation procedures within Net Health.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Valid state license as a Physical Therapist, degree in Physical Therapy or equivalent, at least 2 years of experience in a SNF environment, experience with documenting in Net Health, ability to start by the end of October or November, and flexible to options when submitting application
• Preferred Qualifications: Previous experience in skilled nursing facilities
• Additional Information: The role requires documentation of patient progress, collaboration with the interdisciplinary team, and adherence to facility protocols and procedures.
• Float: Float rotation may be required.
