Job Description

We are seeking a dedicated Registered Nurse (RN) to join our Skilled Nursing Unit. This position requires a commitment to providing high-quality care to patients in a supportive environment.

Unit/Department Overview

The position is located in a Skilled Nursing Unit as part of an acute hospital. The unit features semi-private rooms and relies on an interdisciplinary team to deliver comprehensive patient care.

Responsibilities

• Administer medications and provide wound care, trach care, and IV care.
• Assist with the care of patients with pulmonary diagnoses requiring oxygen therapy, Alzheimer's disease, and dementia.
• Participate in team conferences for acute inpatient rehab.
• Collaborate with Respiratory Therapy for stable vent and trach care.
• Work effectively within an interdisciplinary team that includes PT/OT/SLP, pulmonary or cardiac rehab, case managers, social workers, and dieticians.

Qualifications

• Minimum of 2 years of nursing experience, particularly in Skilled Nursing Facility (SNF) or Subacute care.
• Required certifications: ACLS (AHA), BLS (AHA), and NIHSS - AHA or APEX.

Work Schedule

• 12-hour shifts, primarily during the day.
• Every other weekend required.
• Holidays are also part of the schedule.

Additional Information

• No self-scheduling or block scheduling is available.
• Floating to Acute Rehab and MedSurg/Tele units may be required.
• Appropriate scrubs must be worn: Navy blue for RNs and LPNs, forest green for CNAs.
• Unit-specific orientation will be provided for new hires.
• Use of PAPR or N95 for appropriate situations is enforced.

Patient Care Environment

The patient-to-nurse ratio is 5:1, ensuring a focus on delivering personalized care.
