Job Description

Position: Medical Social Worker (Critical Care)

Shift:

• Day Shift: Monday–Friday, 8:30 AM – 5:00 PM
• 14 weeks contract. 

Responsibilities:

• Provide social work services for patients in the Medical Intensive Care Unit (MICU).
• Perform early identification and high-risk/readmission screening for patients.
• Provide counseling to help patients adjust to illness.
• Conduct safety assessments.
• Coordinate and facilitate discharge planning.
• Identify and address obstacles and barriers to safe discharge.
• Collaborate with the interdisciplinary healthcare team to ensure continuity of care.

Requirements:

• Recent hospital experience in Critical Care units.
• Experience using Epic EMR.
• Active Maryland LMSW or LCSW-C license or an equivalent active out-of-state social work license.
• Candidates with an equivalent out-of-state license must have a completed Maryland licensure application on file with the Maryland Board to be eligible to work.

Schedule:

• Monday–Friday
• 8:30 AM – 5:00 PM

Guaranteed Hours:

• Guaranteed hours as specified by the contract.
