Job Title - LMSW (Licensed Masters of Social Worker)
Unit - Social Services
Duration - 4 Weeks with possibility of extension
Shift - Day 5x8-Hour (08:00 - 16:30)

Position Summary:
Licensed Master Social Worker (LMSW) with experience providing comprehensive psychosocial assessments, care coordination, crisis intervention, counseling, and case management services across diverse healthcare settings.
Job Duties:
Note: Additionally, they need to have experience making referrals to lower level care facilities such as skilled nursing facilities, long term acute care, assisted living facilities and adult family homes.
