Responsibilities
- Conduct psychosocial assessments for patients and families to guide care planning
- Develop and coordinate discharge plans, ensuring appropriate post-acute services and community supports
- Identify psychosocial barriers to treatment, safety, and adherence; connect patients with resources
- Collaborate with the interdisciplinary team, participate in care conferences, and advocate for patient and family needs
- Provide crisis intervention when needed and document assessments and plans in the electronic health record
Requirements
- Active Georgia LCSW license
- Minimum 2 years of experience in a Medical Social Work specialty
- Acute care hospital experience or equivalent in a high-demand inpatient setting
- Ability to work night shifts within a fast-paced care environment
- Strong communication, collaboration, and documentation skills
