Responsibilities
- Conduct psychosocial assessments for patients and families and contribute to care planning.
- Coordinate discharge planning and transitions to home-based services; collaborate with the clinical care team.
- Identify psychosocial barriers to care and connect patients with appropriate community resources and supports.
- Maintain accurate documentation of assessments, plans, and progress in patient records.
- Communicate effectively with clinicians, patients, and families; advocate for patient needs and safety.
- Ensure compliance with applicable regulations and standards; participate in care conferences and case reviews as required.
- Behavioral health experience will be rejected for this assignment.
Requirements
- Licensed Social Worker in New Jersey (LSW) active and in good standing.
- Acute hospital experience required on MedSurg and ED units.
- Discharge planning experience.
- Ability to work in a home healthcare travel assignment setting; flexible to weekend rotation as part of coverage.
