Key Responsibilities

• Evaluate and treat patients with speech, language, cognitive, and swallowing disorders in a skilled nursing facility setting
• Develop and implement individualized treatment plans based on patient needs and goals
• Document patient progress and maintain accurate clinical records using Rehab Optima (Net Health)
• Collaborate with interdisciplinary team members to coordinate patient care

Required Qualifications

• Active California Speech Language Pathologist (SLP) license required
• Minimum 1 year of SNF (Skilled Nursing Facility) experience
• Proficiency with Rehab Optima (Net Health) documentation platform

Preferred Experience / Skills

• Experience working in a fast-paced skilled nursing or post-acute rehabilitation environment
• Strong clinical documentation and time management skills
