St. Joseph Manor Health Care is looking for a compassionate and experienced Speech Language Pathologist to provide comprehensive speech therapy services in a long-term care and skilled nursing environment. The ideal candidate has experience treating adults with speech, language, cognitive, and swallowing disorders and is committed to delivering resident-centered care.

This position offers a full-time day schedule with no on-call or holiday requirements.

Roles & Responsibilities
Evaluate residents with speech, language, voice, cognitive, and swallowing disorders.
Develop individualized treatment plans with measurable short- and long-term goals.
Provide skilled therapy services to improve communication, cognition, and swallowing function.
Perform dysphagia assessments and treatment, including modified barium swallow recommendations when appropriate.
Monitor resident progress and adjust treatment plans based on clinical outcomes.
Educate residents, families, and caregivers on communication and swallowing strategies.
Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary teams.
Participate in care conferences and discharge planning.
Recommend and train residents on communication and swallowing assistive devices.
Complete timely and accurate clinical documentation.
Ensure compliance with Medicare, Medicaid, and facility documentation standards.
Participate in quality improvement initiatives and staff education programs.
Maintain infection control practices and therapy department standards.
Required Qualifications
Master's Degree in Speech-Language Pathology
Active Massachusetts Speech-Language Pathologist License
Certificate of Clinical Competence (CCC-SLP)
Minimum 3 years of Speech Therapy experience
Strong documentation and communication skills
Ability to work independently and within an interdisciplinary team
