Tell us about the unit(s):

· Number of Beds -60 bed acute care and 2 outpatient clinics

· Number of Staff – 19

· Type of staff:

o Day Shift – 3 PTs, 3 PTAs, 2 OTs, 1 COTA, 1 SLP

o Night Shift -

· Patient Ratios – 9-11 patients per day per FTE

· Type of equipment – PT modalities, pool, cardiovascular, Biodex balance, fitness center

· EMR – Acute care: CERNER Outpatient: Nethealth Xfits (formerly ReDoc)

· Outpatient and Acute Care Experience

Preferred or nice to have : candidates with these skills will be considered first.

· Orthopedic/Sports Med/Spine/manual therapy

· Vestibular/vertigo/stroke

List typical procedures performed on unit(s):

· Ther ex, manual therapy, vestibular rehab, balance training, gait training

REQUIRED CERTIFICATIONS:

· Licensed as PT in Client

· Scrub Color/Attire - Acute care: Purple scrubs (eggplant) Outpatient: Business attire or polo-khakis

Unit Guidelines/Policies : Please feel free to attach unit policies and guidelines.

Weekends and On call(if any)

· Rotation every 5 th weekend. Only see priority patients on Saturday average 2-3 hrs. No Sunday visits
