Job Title: Speech and Language Pathologist Profession: Speech and Language Pathology Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1 year of SLP experience required; LTC or Rehab experience required License: Oklahoma State License required Certifications: CCC certified SLP required; New Grads okay with Level II experience Must-Have: - Active ASHA and State Licensure Description: Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Assesses patient progress and adjusts treatments according to regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided according to regulatory and clinical requirements. Documents all required information in the electronic documentation system. Maintains patient records according to policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Participates in required trainings, in-services, and conferences. Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support CFY or Student Speech-Language Pathologists. Adheres to ASHA and state-required practice acts, codes of conduct, and ethics during professional practice as a Speech-Language Pathologist.
