Job Title: Speech and Language Pathologist Profession: Speech and Language Pathologist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours, M-F (Times are flexible as long as consistent and communicated with the Director of Rehabilitation) Experience: 1 year of SLP experience required; LTC or Rehab experience required; New Graduates okay if they have Level II experience License: Texas State License required Certifications: CCC certified SLP required; 3 years of experience holding active CCS preferred Must-Have: - Adherence to ASHA and state-required practice acts, code of conduct, and ethics during professional practice as a Speech-Language Pathologist Description: Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, as well as families, caregivers, and other service providers related to speech disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services rendered per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including, but not limited to, patient care conferences, utilization review meetings, and rehabilitation meetings. Participates in facility and company-required training, in-services, and conferences. Able to obtain necessary Durable Medical Equipment or supplies per regulatory and clinical practice requirements. Able to supervise and support CFY or student Speech-Language Pathologists.
