Start Date: September 21, 2026

Location: Seaside, Oregon and surrounding coastal areas

Schedule: Day shift, 4x8-hour shifts (08:00 - 16:30). Home health setting with 1:1 patient ratios, visiting up to 6 patients per day with a standard of 30 productivity points. No weekends or on-call required.

Requirements:

• Active Oregon state license for Speech-Language Pathology
• Minimum 1 year of skilled home health experience required
• OASIS experience required
• Basic Life Support (BLS) certification
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required
• Experience with EPIC Remote Client for Home Health and Rover phone app preferred
• Two professional references covering 1 year within the last 3 years
