Key Responsibilities of the Speech Language Pathologist- Conduct a minimum of 30 visit equivalents per week
- Travel approximately 60-80 miles daily to visit patients in the outlined regions
- Utilize Home Care Home Base EMR system for documentation
- Work collaboratively with multi-disciplinary team
- Provide patient care for conditions such as cognitive challenges, trach care, swallowing difficulties, stroke recovery, and aphasia
- Effectively manage time and independently schedule patient appointments
Requirements of the Speech Language Pathologist- 1 year of Home Health experience is required
- Valid Driver's License and proof of Insurance required
- Experience with Homecare Homebase EMR is essential
