- Assess, diagnose, and treat residents with speech, language, cognitive-linguistic, voice, and swallowing disorders. - Develop and implement individualized therapy and treatment plans to improve communication, cognitive function, and safe swallowing. - Perform and interpret swallowing assessments and recommend diet modifications as needed. - Provide direct therapy services to residents to enhance functional independence and quality of life. - Document all assessments, treatments, progress, and discharge notes in compliance with regulatory and facility standards. - Educate residents, families, and staff about therapy plans, communication techniques, and safe swallowing strategies. - Participate in interdisciplinary care planning and team meetings to support resident-centered care. - Monitor resident progress and adjust treatment plans as necessary to achieve optimal outcomes. - Ensure compliance with professional, legal, and regulatory requirements. - Supervise Clinical Fellows and/or Speech-Language Pathology Assistants if applicable. - Work collaboratively within a skilled nursing/long-term care environment, often with residents who have complex medical needs.
