Job Summary (List Format) Speech-Language Pathologist (SLP):
- Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in skilled nursing facility residents.
- Develop and implement individualized treatment plans to promote residents' functional independence, safety, and quality of life.
- Provide direct therapy for speech, language, cognitive, and swallowing needs.
- Conduct and interpret comprehensive evaluations and swallow assessments; recommend diet modifications as needed.
- Document assessments, treatments, progress, and discharge summaries per regulatory guidelines.
- Educate residents, families, and staff on therapy plans, communication techniques, and safe swallowing practices.
- Participate in interdisciplinary care team meetings and contribute to resident-centered care planning.
- Monitor resident progress and adjust treatment plans accordingly.
- Ensure compliance with professional, legal, and regulatory standards.
- Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs), if applicable.W
