Speech Language Pathologist (SLP)

Key Responsibilities- Provide speech-language pathology services to home health patients across an 11-county territory spanning two office locations
- Evaluate and treat patients with diagnoses including CVA and Aphasia
- Manage a caseload of approximately 6 patients per day in a home health setting
- Travel throughout the assigned service territory to conduct in-home patient visits
- Maintain professional documentation and reporting using provided phone and tablet technology
- Participate in a two-week department-specific orientation with a designated work coach
Required Qualifications- Active Kentucky state license in hand at time of submission — no pending licenses accepted
- CCC-SLP credential required
- Current BLS certification required
- Minimum of 1 year of clinical experience
- Prior home health experience required
- Ability and willingness to travel across an 11-county territory, with corner-to-corner travel exceeding 2.5 hours
Preferred Experience / Skills- Experience treating CVA and Aphasia patient populations
- Familiarity with home health documentation and care coordination workflows
