Contract | Days | 5x8 Hours (8:00 AM – 4:30 PM)

Key Responsibilities

• Provide speech-language pathology services to patients in a home health setting
• Complete OASIS assessments accurately and in accordance with home health requirements
• Develop and implement individualized treatment plans based on patient evaluations
• Document patient progress and clinical notes using required charting systems
• Collaborate with interdisciplinary care teams to coordinate patient care
• Float to other units or locations based on facility needs as requested

Required Qualifications

• Active Speech Language Pathologist license; Virginia license required to be in hand at time of submittal or valid compact license with proof provided
• OASIS experience required
• Home health experience required
• Specialty experience within the last year required
• BLS certification through the American Heart Association (AHA) required
• SSN required at time of submittal

Preferred Experience / Skills

• Experience with EPIC electronic health records system
• Prior experience in a travel or contract therapy role
• Strong communication and patient education skills across diverse patient populations
