Speech Language Pathologist
LocationGloucester, VA
Schedule / Job TypeContract | Days | 5x8 Hours (8:00 AM – 4:30 PM)
Key Responsibilities- Provide speech-language pathology services to patients in a home health setting
- Complete OASIS assessments accurately and in accordance with home health requirements
- Develop and implement individualized treatment plans based on patient evaluations
- Document patient progress and clinical notes using required charting systems
- Collaborate with interdisciplinary care teams to coordinate patient care
- Float to other units or locations based on facility needs as requested
Required Qualifications- Active Speech Language Pathologist license; Virginia license required to be in hand at time of submittal or valid compact license with proof provided
- OASIS experience required
- Home health experience required
- Specialty experience within the last year required
- BLS certification through the American Heart Association (AHA) required
- SSN required at time of submittal
Preferred Experience / Skills- Experience with EPIC electronic health records system
- Prior experience in a travel or contract therapy role
- Strong communication and patient education skills across diverse patient populations
