Contract | Part-Time | Days | 2 days per week, 8-hour shifts (8:00–16:30)

Key Responsibilities

• Provide speech-language pathology services to patients in a home health setting
• Complete and maintain OASIS assessments in accordance with home health requirements
• Develop and implement individualized treatment plans based on patient evaluation and clinical findings
• Document patient progress and maintain accurate clinical records
• May be requested to float based on facility needs

Required Qualifications

• Active Speech Language Pathologist license; Virginia license required to be in hand at time of submittal or proof of compact license
• OASIS experience required
• Home health experience required
• Specialty experience within the last year required
• SSN required at time of submittal
• One supervisory reference required at time of submittal
• Local candidates are welcome and accepted at the same rate

Preferred Experience / Skills

• Experience with electronic health record documentation
• Strong communication and patient education skills
• Ability to manage a flexible schedule with minimal time-off requests during assignment
