Key Responsibilities- Perform evaluative and therapeutic procedures for communication disorders and dysphagia, including modified barium swallow studies, under physician or accepted healthcare provider orders.
- Evaluate residents per department protocols and establish individualized therapeutic plans of care with functional long- and short-term goals, including frequency, duration, and rehabilitation potential.
- Monitor and progress treatment programs toward established functional goals, modifying plans of care as necessary and documenting all interactions per department procedures.
- Provide consultation to referring physicians and interdisciplinary care team members regarding therapeutic and discharge needs; attend all pertinent team meetings.
- Recommend and obtain appropriate communication and swallowing assistive devices, ensuring residents and caregivers receive proper instruction and education.
- Provide in-services to nursing and other departments; actively assist with the development of new Speech Therapy programs and services.
Required Qualifications- Master's degree in Speech-Language Pathology required.
- Active Speech and Language Pathologist license required.
- Certificate of Clinical Competence (CCC-SLP) required.
- Strong verbal, written, and documentation communication skills.
- Ability to work collaboratively within an interdisciplinary team environment.
- Understanding of Medicare, Medicaid, and other reimbursement regulations.
Preferred Experience / Skills- Basic Life Support (BLS) certification preferred.
- Prior experience in a long-term care or skilled nursing facility setting preferred.
