Key Responsibilities
- Evaluate and treat patients with speech, language, cognitive, and swallowing disorders in a skilled nursing facility setting
- Develop and implement individualized treatment plans based on patient needs and goals
- Document patient progress and maintain accurate clinical records using Rehab Optima (Net Health)
- Collaborate with interdisciplinary team members to coordinate patient care
Required Qualifications
- Active California Speech Language Pathologist (SLP) license required
- Minimum 1 year of SNF (Skilled Nursing Facility) experience
- Proficiency with Rehab Optima (Net Health) documentation platform
Preferred Experience / Skills
- Experience working in a fast-paced skilled nursing or post-acute rehabilitation environment
- Strong clinical documentation and time management skills
