Speech Language Pathologist (SLP) – Long Term Care (LTC)

Key Responsibilities
  • Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders; develop appropriate plans of care following all regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions and adjust treatment plans based on patient progress.
  • Evaluate treatment outcomes for effectiveness and make referrals or modify services accordingly; provide consultation and counseling to patients, families, caregivers, and other service providers.
  • Accurately document all regulatory and clinically required information in the electronic documentation system, including treatment minutes, progress notes, and comprehensive discharge summaries.
  • Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings, as well as facility and company required trainings and in-services.
  • Supervise and support Clinical Fellowship Year (CFY) or student Speech-Language Pathologists as needed; obtain necessary DME or supplies per regulatory and clinical practice requirements.
Required Qualifications
  • Active Oklahoma State Speech-Language Pathologist license.
  • Certificate of Clinical Competence (CCC) from ASHA required.
  • Minimum of 1 year of SLP experience, or completion of at least a Level II clinical placement (new graduates with Level II experience will be considered).
  • Long Term Care (LTC) or rehabilitation setting experience required.
  • Adherence to ASHA and state-required practice acts, code of conduct, and ethics standards.
Preferred Experience / Skills
  • Experience supervising CFY or student Speech-Language Pathologists.
  • Familiarity with electronic documentation systems used in LTC or rehabilitation environments.
