Description:
The Home Care Scheduler is responsible for coordinating the scheduling and tracking of Home Care and Home Care Assistant assignments.
The Home Care Scheduler will provide outstanding customer service to all stakeholders and maintain current and accurate records of all scheduled appointments.
This role requires effective coordination of home care personal care assignments as directed by the Home Care Supervisor.
Adjustments to schedules based on changes must be communicated with all stakeholders, including participants and family.
It is essential to ensure required authorizations in the electronic medical record for approved personal care.
The Home Care Scheduler must ensure all Care Logs and Timesheets are received from vendors and in-home staff visits.
Procedures must be consistently implemented to ensure a successful experience for providers, staff, and participants.
This includes making reminder calls, providing instructions, or sending materials ahead of time.
Current and accurate records of all scheduled home care personal care assignments must be maintained.
Records should include both scheduled times and outcomes, indicating whether an appointment occurred or did not occur.
Flexibility is required, as the scheduler may need to work varied schedules that can include evenings and weekends.
All required documentation must be completed in a timely and accurate manner.
Privacy must be protected, and confidentiality of all company procedures and information must be maintained.
Participation in any required Quality Improvement activities, staff training, and meetings is required.
Regular communication with the Supervisor and Home Care Coordinators regarding workload and priorities is essential.
Timely completion of all mandated trainings and education is necessary.
This role includes flexibility in performing assignments as business needs evolve.
Other duties may be assigned as needed.
